  • Proof of address dated within the last 6 months, if your current address is not recorded on your lost, stolen, or damaged licence, and differs from the one provided when you obtained your Public Services Card.

  • A Driving Licence Eye Test Report Form, if required (typically necessary for certain age groups or medical conditions).
